Issues and Progress - When I entered Always to 4.1.1 to all tasks,...

Issue Title

When I entered Always to 4.1.1 to all tasks, NO to the main question in 4.1.2 and NO to the main question in 4.1.3, then it missed the category question.

Further Information

doesnt seem possible. This would be scenario C below after 4.1.1? straight to 4.1.2?

instructions say ifor 4.1.2:

A: If the answer is ALWAYS or SOMETIMES to P) AND Q) then go to Question 4.1.3. 'bmkOffBed_4_1_2_300 bmkOffBed_4_1_2_310
'        B: If the answer is ALWAYS or SOMETIMES to ANY of the tasks listed above and NEVER to EITHER K) or l) then the individual cannot walk, go to Question 4.1.4.
'        C: If the answer is NEVER to ALL of the criteria, the individual requires full support.   Go to Section 4.2.
